Set Custom Roles should save. Every time the server is rebooted, I have to redo my roles.

Most keys should spawn in random locations, not just the Principal's Office key.

A "Random" Game Mode option that will choose one of the others automatically when the game begins.
This way, there's no possible way to know what Game Mode it is, thus preventing Metagaming in this game mode.
You wont know if it was an average killer, a witch, Kira, or a Shiki.

Red Box (the glowing one)
Use the Red Box. The scissors will combine into a giant pair of scissors that deal massive damage. This requires the rare ass spawning of three Red Boxes though.

The Fire Extinguisher should have an actual use other than just blindly spraying.
I recommend adding a way to create fire that can only be put out using a fire extinguisher. The fire will light players on fire, dealing moderate damage over time. It will also burn down doors and destroy any items thrown into the fire. Including keys.

A little note on each thing mentioned:
Set Custom Role Saving - I agree with this because it's convenient for the players.
Keys Spawning at Random Places - Considering that the Principal's Key only does so for a specific reason I don't agree with this. It makes the room with the locker in entirely pointless.
Random Mode: While you'll still know if it was a Kira, having a mode where you've got no idea what it is before it starts brings a RP problem with the body discovery. In Shiki it brings up the question of how our characters missed all three bite marks when at least one should probably be exposed. A random mode like Pyrce's where it shows the mode for a few moments before starting could be a decent idea though.
Rending Scissor Blades: My only issue with this suggestion is that the Rending Scissor Blade is already supposed to be the ultimate secret weapon and even it is difficult to explain in RP.
Fires: Not too sure on this one. I can see how it'd be interesting but the door burning and item destroying seems a little too easy to misuse, especially if one item can be used to do this many things.

Set Custom Roles is SUPPOSED to save, if it is not, that belongs in bug reports. I am surprised that nobody brought it up yet, though when I tested it, it saved for me. I will test again today. Fires are already planned. The Rending Scissor Blades would require only 2 (out of the two that spawn) glowing boxes, not three, in the way you described it. And I have to agree with Bervose with the key spawning. As for the Random mode, the game would tell you what mode it was via the Guides, so there's no problem there, even if the announcements are the same.
Edit: I can confirm that Set Custom Roles is indeed saving for me.

It saves until the server is rebooted, then we all have to redo our custom roles. Once, I got Golden Witch and I was highly confused and we had to restart because I didn't understand the Game Mode. I had that role turned off prior to the reboot. Wasn't aware this was a bug. Figured it was just a feature that we didn't have, like how Watcher icons don't save after a reboot.

Eh, I guess I may put in the music player today after fixing up Madoka, since it's becoming a wide suggestion, even though only hosts/minihosts/admins would be able to use it to play to the whole world, and I would have to make ambience have its own sound channel, volume settings, and Play proc.

Both of those are up to Frosty, and I believe that he discussed it yesterday, the uniforms at least. I thin he said that Red is too reminiscent of Pryce and he wants to keep it in the spirit of Mitadake.

Maybe the teacher in the courtyard should have a model update or be replaced by the Principal? She looks out of place and the Principal's model is already seen in his picture on the wall saving time.
